The song is written as a soft acoustic ballad. However, the lyrics are of a destructive relationship between Yankovic and an unnamed girl. Although they were formerly in love, the "flames died down" and they are no longer passionate.
Much of the humor of the song is derived from the stark contrast between Yankovic's delivery and the nature of the lyrics. As the relationship deteriorates, the woman's actions increase from mere adultery (sleeping "with the whole hockey team"), to bodily harm and pranks (shaving his eyebrows while he slept), to attempted homicide (drilling a hole in his skull and leaving him for dead). All of these actions merely leave Yankovic wondering about the future of their relationship, and with a naive quizzical nature guessing "you don't love me anymore".
The song's length on "Off the Deep End" is over 11 minutes long. However, after the 4 minute song, there are 7 minutes of silence, before about 8 seconds of strange notes playing, violent noises in the background, and Yankovic screaming. After this the song ends. This is most likely the girl attempting to kill him again (he did say "please put down that knife" in the song, although the violent sounds sound more like a chainsaw.) This could be a parody of Nirvana's silence at the end of Nevermind before the band launches into the last uncredited song.
Although the video is a parody of Extreme's More Than Words, Weird Al's singing style in the song sounds more like a parody of James Taylor.
The song was a big hit in Asia and reached the number one spot in the Philippines for 3 weeks during the last quarter of 1992. It was one of the biggest hits of that year.

The music video is a parody of "More Than Words" by Extreme, and continues where the "Smells Like Nirvana" video ends.
Within the music video:
- The amplifier is turned on full blast. The bass player knocks it over instead of simply turning it off.
- The drummer is dressed like Dave Grohl from Nirvana. He drops the drumsticks instead of just putting them down, and takes his wig off.
- Yankovic appears as Gary Cherone.
- Yankovic gets a guitar, but as soon as he's going to start playing it, he puts it away.
- The drummer and the bassist appear sitting on couches near Yankovic and the guitarist. The bass player has a pig on his lap instead of a dog, and by the end of the video, eats it. The drummer uses a blowtorch as his lighter, but catches fire.
- Bassist Steve Jay actually chipped a tooth as he bit into the pig, and he noticeably winces as this happens.
- Two musicians not from the original video appear: a pianist (Robert Goulet) and a cellist (the janitor from the "Smells Like Teen Spirit" and "Smells Like Nirvana" videos). The cellist accidentally throws his bow into the pianist's eye and the pianist gets his hand cut off, latter he is shown with a bear trap on his foot.
- At the end, Yankovic grabs the guitar and smashes it to pieces.
